If you’re applying for a license or clearance with California’s Community Care Licensing Division (CCLD), your Live Scan fingerprinting fee will vary based on your facility or job type.
The California Department of Justice (DOJ) charges a government fee specific to each DOJ Abbreviated CCLD Facility Type, and Anshin Notary charges a $30 fingerprint rolling fee in addition to that.

All CCLD applicants must complete the “Request for Live Scan Service” (LIC 9163) form:
- Box 3: Enter the correct DOJ Abbreviated Facility Type from the table above.
- Box 2: Select your working title (e.g., Employee, Volunteer, License Applicant, etc.).
- Bring 2 copies of the form when you arrive at our office for fingerprinting.
