You should complete your Live Scan Service Form prior to your visit. But many don’t do so. Some have it but it’s not filled out correctly. Or some have it but it’s not printed out. Check out our post if you want to learn how many forms you need to print out and bring to your appointment. 

Filling out the Live Scan Service Form

Your employer, or whoever is asking you to get fingerprinted, needs to fill out the top of the form. The Live Scan Service From here is not the correct form. You can’t have a generic form. Your requesting agency, or employer, needs to give you the correct form that is specific to it. They should not leave it to you to get the form. You can have them call us to explain the process or we can  call them to explain. 

For most forms, you need to complete the information in the middle of the form. This consists of personal information of the type that would be found on a drivers license.

The most important information in the form is in the top 2 sections, which contain the “ORI Number” and the “Applicant Type”, in addition to other information.  Ordinarily, each agency which requires the client to obtain a Live Scan has multiple Applicant Types registered with the CADOJ. Thousands of Agencies are registered with the California Department of Justice and authorized to receive electronic copies of individual applicant’s criminal history records.

We will complete the bottom of the form. The most important piece of information there is the ATI Number. Each live scan record has a unique ATI number. Most agencies want applicants to send them the completed form, so they have a copy of the ATI number.

The agency requesting your background check must provide you with a “Request for Live Scan Service” form. They are responsible for completing the sections labeled “Contributing Agency Information,” “Applicant Submission,” and, if applicable, “Employer” and “OCA number.” You, the applicant, must fill out the “Applicant Information” section. Bring the completed paper form with you to your Live Scan appointment.

You do not submit the form yourself. Instead, bring the completed Live Scan request form in paper format to your Live Scan provider. We will scan your fingerprints, complete the submission electronically to the California DOJ (and FBI, if applicable), and return two copies of the form to you—one for your records and one for the requesting agency if they require it.

No. The Live Scan form is not available for online submission or upload. The requesting agency must give you a printed copy with their information already filled in. You must hand-carry this form to your Live Scan appointment, where we use it to complete the fingerprint submission.

Yes. You must bring the completed Live Scan request form and one valid, government-issued photo ID. Acceptable forms include a California driver’s license, a California identification card, or an out-of-state driver’s license. U.S. passports are also accepted. The ID must be current and not expired.

Most electronic Live Scan submissions are processed by the California DOJ within 72 hours, but delays can occur due to manual review, incorrect form information, or other factors. You will not receive results directly—only the requesting agency will. If you haven’t heard back after 7 days, contact the agency or use your ATI number to check the status on the DOJ website.
