Live Scan Fees for Community Care Licensing (CCLD) Applicants
If you’re applying for a license or clearance with California’s Community Care Licensing Division (CCLD), your Live Scan fingerprinting fee will vary based on your facility or job type.
The California Department of Justice (DOJ) charges a government fee specific to each DOJ Abbreviated CCLD Facility Type, and Anshin Notary charges a $30 fingerprint rolling fee in addition to that.

📄 How to Fill Out Your LIC 9163 Form
All CCLD applicants must complete the “Request for Live Scan Service” (LIC 9163) form:
- Box 3: Enter the correct DOJ Abbreviated Facility Type from the table above.
- Box 2: Select your working title (e.g., Employee, Volunteer, License Applicant, etc.).
- Bring at least one printed copy of the completed form. We are required to keep one, so bring a second if you want a copy for your own records, and a third if your agency wants physical paper as well.

Questions We Get From Caregivers
I was told to get fingerprinted for a caregiving job and I do not know which applicant type I am.
This is where most caregiver appointments go wrong, and it is worth settling before you arrive. Community Care Licensing covers several distinct categories and they are not interchangeable. A caregiver working in a licensed single-family home is not the same applicant type as someone working in a residential care facility, and facilities serving children are handled separately from those serving adults. IHSS is a different programme again, with its own form and its own pathway.
The correct applicant type is printed on your form. If it is blank, or you are not certain which one applies to you, text or email us a photo of the form before your appointment and we will confirm it. Two minutes by text beats sitting in our office trying to reach somebody at your agency who is not picking up.
How do I know I have the right form?
Two things to check before you come in. The form should be the LIC 9163, which is Community Care Licensing’s version of the Request for Live Scan Service rather than the generic BCIA 8016. And the ORI printed on it should read A0448, which is the code CCLD submissions go under. If the ORI reads anything else, you either have a form from a different agency or the wrong form altogether, and the submission will not reach Community Care Licensing.
What happens if the applicant type is wrong?
The submission goes to the wrong place, or it is rejected, and you pay the government fee again to redo it. The fingerprints are fine; the routing is not. That is why we would rather read your form first than fix it afterwards.
